Chad was born in Bismarck, North Dakota and grew up in Portland, Oregon. He attended Harvard School of Dental Medicine before leaving to pursue medical school at Oregon Health and Science University in Portland.
Chad completed his residency in urologic surgery at Oregon Health and Science University.
In Chad’s free time he enjoys spending time with his wife and son, snow skiing, hiking, and mountain biking.
